FZ GO KART - KORONIOS GEORGIOS

In consideration of my participation in the go-kart and all associated activities and events
operated by <FZ GO KART-KORONIOS GEORGIOS>, I hereby release <FZ GO
KART-KORONIOS GEORGIOS>, its members, employees, contractors, agents,
successors and other entities working for or with it from all liability, including but not limited
to all liability caused by the only or partial fault of <FZ GO KART-KORONIOS
GEORGIOS>, loss, costs, damages, and causes of action, foreseen and unforeseen,
including but not limited to injury to the person or property and emotional and bodily injuries,
as well as death, deriving from or related to my renting and operation of a go-kart at <FZ
GO KART-KORONIOS GEORGIOS>
I understand that by engaging in the activities offered by <FZ GO KART-KORONIOS
GEORGIOS> I can be exposed to various inherent risks and dangers. I voluntarily accept
and assume all inherent risks and dangers, including the possibilities of emotional and bodily
injuries, death, losses and damages, including but not limited to property damage, resulting
from my participation in any activities offered by or connected to <FZ GO KARTKORONIOS
GEORGIOS>. I also agree to pay <FZ GO KART-KORONIOS
GEORGIOS> for all damages to the property, facilities and equipment caused by me,
foreseen and unforeseen.
This release shall continue in force each time I rent and operate a go-kart at <FZ GO
KART-KORONIOS GEORGIOS>. This release shall be binding upon myself, my family,
my representatives, agents, assigns, successors and heirs.
By signing this acknowledgement of risk and release of liability waiver, I certify that:

A) I have read the content of this acknowledgement of risk and release of liability waiver
form, I understand it and sign it freely;
B) I am at least 18 years of age;
C) If I am under 18 years of age, my parent of legal guardian must sign this form;
D) I have not consumed alcohol or drugs;
E) I am not pregnant;
F) I will follow all rules and instructions of <FZ GO KART-KORONIOS GEORGIOS>;
G) I am aware I am waiving certain legal rights including the right to sue.

Considering my participation in Go-karting and all related activities and events conducted by , I hereby release , its members, employees, contractors, agents, successors and other entities working for or cooperating with it from all liability, including, but not limited to, any liabilities arising from its fault or partial fault of , loss, costs, damages and causes of action, foreseeable and not, including, but not limited to, personal injury or property damage and mental or physical injury as well as death, arising from the rental and operation of go-karts at . I understand that by participating in the activities offered by I may be exposed to various inherent risks. I voluntarily accept and assume responsibility for all potential risks, including, but not limited to, the possibility of mental and physical injury, death, losses and damages, including, but not limited to, property damage, arising from my participation in any activities offered by or associated with . I also agree to pay for any damages to property, facilities, equipment caused by me, whether foreseen or not. The liability waiver continues to apply whenever I rent and operate a go-kart at . The liability waiver is binding on me, my family, my representatives, my agents, my assigns, my successors and my heirs. By signing this risk acknowledgment and liability waiver, I certify that: A) I have read the contents of this risk acknowledgment and liability waiver, I understand it and I am signing it of my own free will. B) I am at least 18 years old. C) If I am under 18 years old, my parent or legal guardian must sign this declaration. D) I have not consumed alcohol or drugs. E) I am not pregnant.
